Personal Advisor, Telford and Wrekin 
Pay rate – £21.25ph 
 
Dependent on visits etc, the teams are generally based in the office 2 times a week. Location: Darby House, Lawn central, Telford, TF3 4JA & Leavers Lounge, Meeting Point House, Southwater, Telford, TF3 4HA Job Responsibilities: – Undertake the statutory role of Personal Adviser role for individual Care experienced young people from age 16 up to the age 21 or 25. – To be the key professional and case manager to Former Relevant care experienced young people and when needed to those young people who are Relevant or Qualifying for advice and assistance – To work with young people leaving care and their support network to support the development of independent living skills and ensure a smooth transition to independence – To assist and support young people to prepare for leaving care and living independently; providing support, advice and guidance to them up to age 25.
