Experienced HPCP registered Occupational Therapist required to work in the Adults and Older People’s Review Team carrying out annual review and reassessments. Evaluate current care packages and recommend changes where required, work collaboratively with multidisciplinary teams, carers, and partners, maintain accurate records and uphold professional standards. 
£35.38 per hour.
