Enablement Team

What you’ll do:

Complete holistic OT assessments in the community
Deliver goal-focused rehabilitation and enablement programmes
Recommend equipment, adaptations and practical solutions
Support people to maximise independence and reduce reliance on long-term care
Work collaboratively with health, social care and community partners

What we’re looking for:

Qualified Occupational Therapist
HCPC registered
Passion for strengths-based practice and promoting independence
Excellent communication and assessment skills
Experience in community, rehabilitation, enablement or adult social care settings desirable
