Adult’s Social Worker, Hospital Team, Shropshire 
Pay rate – £31.97 per hour 
Complete Care Act Assessments. Undertake Mental Capacity Assessments and Best Interests decisions. Arrange Pathway 3 placements where long-term care is required. Work with individuals, families, and partner agencies to ensure appropriate care and support is in place. Respond to referrals, undertake assessments, and support timely hospital discharges as part of the Hospital Discharge Team. The role is primarily based at Royal Shrewsbury Hospital and requires regular on-site attendance.
