Contract role, Discharge to Assess
 
Pertemps are recruiting for experienced Adult’s Social Workers in Bristol
This is to work in a Hospital Team across 3 Hospitals.
 The Role:

• Assessment and Review
• Post hospital discharge
• CAA and Support planning
• BI
• MCA, Safe Guarding
• Duty cover
• Attend QA panel for funding provision.
