Adult Social Worker – Older People Team

We are currently recruiting for an experienced Adult Social Worker to join the Older People Team in Hertfordshire.

The role will involve:

• Completing Care Act assessments and reviews
• Developing person-centred care and support plans
• Safeguarding and managing complex cases
• Working with individuals aged 65+ and their carers
• Liaising with health, social care and other partner agencies
• Promoting independence, choice and positive outcomes

Requirements:

• Social Work England registration
• Relevant adult social care experience
• Experience working with older people
• Strong knowledge of the Care Act, safeguarding and assessment processes
• Full UK driving licence
