We are currently recruiting for an experienced Adult Social Worker to join a Hospital Team in Merton.
The role will involve working with adults in a hospital setting, supporting assessment, discharge planning and positive outcomes for individuals returning to the community or moving into appropriate care.
Key responsibilities:

• Undertake Care Act assessments and reviews
• Support hospital discharge planning
• Assess care and support needs and develop person-centred plans
• Work collaboratively with health and social care professionals
• Safeguard adults and respond to identified risks
• Liaise with families, carers and partner agencies
• Maintain accurate records and work within statutory timescales

Requirements:

• Social Work England registration
• Qualified and experienced Adult Social Worker
• Experience within hospital or discharge services is desirable
• Strong knowledge of the Care Act and adult safeguarding
• Ability to manage a busy caseload effectively
