Adult Social worker, Hospital Pathway Team, Sutton  
Duties includes: – Carrying a case load. – SW will be allocated Safeguarding cases to triage, Safeguarding Adults enquiry planning, triage, arranging multi disciplinary meetings, – Completing Risk assessments in community , Nursing homes, care homes. – Carrying out Mental capacity assessment and best interest meeting for long term care and support arrangement. – Carrying out Continuing health care assessment, jointly with professionals from ICB. – Carrying out Care act assessment. – Liaising with other professionals and residents family members. Liaising with housing, – Carrying out Court of protection application for deputyship, Sec 21 A procedures for the allocated cases. – Carrying out Duty responsibility 2 days a month. – The role is hybrid working 3 days office based at Civic offices in Sutton, and 2 days home working.
