Adult Social Worker – Hospital Discharge Team
Location: Aylesbury
Rate: £36–£37 per hour
Working Pattern: Hybrid – 2 days onsite

Pertemps are currently recruiting for an experienced Adult Social Worker to join a busy Hospital Discharge Team based in Aylesbury.

This is an excellent opportunity to play a key role in supporting timely and effective hospital discharges, working closely with health professionals, service users, families and partner agencies to ensure safe and appropriate outcomes.

Key Responsibilities

  • Managing a complex caseload within a fast-paced Hospital Discharge Team
  • Completing Care Act assessments, reviews and support planning
  • Assessing service users to identify appropriate care and support arrangements
  • Supporting safe, timely and effective hospital discharges
  • Working collaboratively with multidisciplinary teams, including NHS colleagues
  • Liaising with families, carers and external agencies
  • Completing Mental Capacity Assessments and Best Interest decisions where required
  • Ensuring safeguarding concerns are identified and appropriately managed
  • Maintaining accurate and timely case records

Requirements

  • Qualified Social Worker with Social Work England registration
  • Previous experience within Adult Social Care
  • Experience of hospital discharge or a fast-paced statutory social work environment
  • Strong knowledge of the Care Act 2014 and relevant adult social care legislation
  • Ability to manage a complex caseload and work to deadlines
