Adult’s Social Worker, Transitions Team, Lancashire 
Pay rate – £35 per hour 
Contract role, Young Adults and Transitions Team 
 
Pertemps are recruiting for an experienced Adult’s Social Worker in Lancashire 
Within this team practitioners will support young people as they prepare for adulthood and transition into adult services. The focus of the role is to ensure that the right support is in place at the right time, working collaboratively with partners, families, and young people to promote independence, build confidence, and enable a smooth and well-coordinated transition into adult life. 
 
There is plenty of hybrid working available.
