Pay rate £32 to £35 per hour

Pertemps are recruiting for an experienced Adult’s Social Worker in Portsmouth. 
This is to work in a Immediate Care Team 

• Your role will involve assessing the needs of adults and their Carers, to identify creative ways to meet their assessed outcomes under the Care Act 2014.  

• We are looking for a social worker to work across all of these areas, so would be the ideal opportunity for someone seeking a variety within their work. One day you may be working with a person in hospital and supporting them with their discharge planning, ensuring a home first approach at each stage, the next day, visiting a person in their own home, supporting them back to independence through the application of the strengths -based approach.  

• The role includes outcome led interventions, using a strength-based approach to support people to live the lives they chose. You will work collaboratively with others, maintaining a strength based and aspirational approach to achieve the outcomes the person wants. 

• Duties include assessing clients’ needs and agreeing a person-centred support plan, completing reviews of assessments and support plans, supporting people with their health, mental health, and social care needs, and assessing mental capacity when required.
