Social Worker – Care Act Assessments

Pertemps is recruiting an experienced Social Worker to join an Adult Social Care team in Aylesbury, focusing on high‑quality Care Act assessments and person‑centred planning. This is a key role supporting adults to maintain independence, safety and wellbeing.

You will:

• Carry out Care Act assessments with adults in the community
• Develop proportionate, strengths‑based support plans
• Work collaboratively with health partners, voluntary services and internal teams
• Ensure clear, timely recording and decision‑making
• Promote independence, choice and positive outcomes for service users

You need:

• Experience completing Care Act assessments within adult social care
• Social Work England registration
• Strong assessment, communication and planning skills
• Eligibility to work in the UK
