Social Worker -Hospital discharge

Qualified Adults Social  Worker -Hospital Discharge- Hammersmith and Fulham 
 
We are looking for  a Qualified Social Worker to join the hospital  discharge team. You will have a variety of duties including discharge to asses , social care plans and Reablement. You will working on a rota basis . You must be registered with the SWE  and have completed your ASYE as well as having 2 years PQE.
