Pertemps are recruiting for qualified and experienced Social Workers to join the Leaving Care Service.

The postholder will manage a caseload of young people who are preparing to leave care. They will also be working with the allocated Pathway Advisors for the young person who are also based within our service. The service links in early with the young people transferring for other services and the postholder will lead on the completion of pathway plan assessments and the initial pathway plan. It will remain the postholder’s ongoing responsibility to maintain the plans in line with the required legislation up until the young person is 18 years old when they will then fully transfer to their allocated Pathway Advisor. In an addition to managing their own caseload the postholder will also be required to be part of the duty system that provides additional support to young people alongside Pathway Advisors.

Candidates are required in office 3 days per week.
