Pertemps are recruiting for qualified and experienced Adult Social Workers to join the Assessment & Intake team. The Assessment & Intake Team is a fast-paced team based in the community and provides an initial assessment function for the Older Persons and Physical Disability Service. The team carries out short term pieces of work – up to 6 weeks. The role includes the provision of a short-term intensive assessment period, assessing the person’s ability to maximise their independence and to provide a robust assessment that measures the amount of ongoing support needed.

Main duties:

• Holding a case load
• Completing Care Act Assessments
• Undertaking unscheduled reviews of care
• Completing carers assessments
• Presenting packages of care to the funding panel for approval
• Using community resources to support service users and their families to maximise their independence
• To ensure that all interventions are strength based
• Completing Mental Capacity Assessments and Best Interest Decision making
• Arrangement of care home placements from the community
• Stabilising complex case issues ready for transfer to the longer-term team
• Undertaking the initial concern stage of Adults Safeguarding.

Flexible working arrangements are in place. Presently the Assessment and Intake Team are operating 2 days in the office and 3 days working from home rota. This enables team members to reduce the amount of commuting (which impacts on work life balance) whilst also ensuring good onsite support and integration with the team and managers.

Requirements:

• At least 2 years post-qualifying experience within a statutory adult social care team, particularly experience of assessing the social care needs of elderly people and people with disability
• SWE registration
