Pertemps have a fantastic opportunity for an experienced Occupational Therapist in Chester, working within a reablement team.

THe role is part of a fast paced team, working with reablement to promote people reaching their goals.

Key Responsibilities:

• Conduct comprehensive assessments to understand clients’ needs and develop personalized reablement plans.
• Implement therapeutic interventions to promote independence and well-being.
• Collaborate with a multidisciplinary team to ensure holistic care.
• Monitor progress and adjust treatment plans as necessary.
• Provide education and support to clients and their families.
