Essential:
– Car driver
– Registration with HCPC
– Experience working in MDT and rehabilitation preferable.
– Some post graduate experience rather than just qualified due to the complexity of the team’s caseload at times. To work across all pathways of Intermediate Care – reablement, rapid response and rehab beds. Must have some community experience. Location: Blakenall Village, Thames Road, Walsall, WS31LZ
