Contract role – Hospital Discharge, CHC, MDTs, Care Home reviews 
 
Pertemps are recruiting for an experienced Adult’s Social Worker in the Fylde area of Lancashire. 
This is to work in the Hospital Discharge support team 
 
There is plenty of hybrid working available.
