Adult’s Social Worker, Immediate Care Team, Southwark

We are advertising for an Urgent Response Social Worker to join Intermediate Care Southwark. The team support Southwark’s @Home/Urgent pathway and is a hospital admission avoidance service. We intervene with residents who can be supported home from hospital emergency departments and who are at risk of admission from home. The team is embedded with health colleagues in a multi-disciplinary team, including Occupational Therapists, Physiotherapists, Social Workers, Assistant Practitioners, Nurses, Doctors, Pharmacy and Rehab Support Workers. The role is fast paced and workers will have a caseload between 5-7 cases and will generally move cases on within 10 days maximum. The role involves putting in place bridging urgent care and working with the MDT to stabilise residents, transferring cases to Reablement, completing Care Act Assessments, Care and Support Planning, Reviewing existing care and support. Workers will also be involved in completing Safeguarding Concerns and Mental Capacity and Best Interests Decision Making. Requirement Essential: SW qualified and registered, with at least 2 years post qualifying experience. Desirable: Experience of work in fast paced service such as Urgent Response or hospital discharge. Desirable: Understanding of Reablement ethos and strengths based practice. Location/nearest travel links: Main Adult Social Care building, 132 Queens Road, London, SE15 2HP. The nearest station is Queens Road Peckham, which is served by the overground and mainline trains. Urgent Response (health) also have bases at Tulse Hill and on the Walworth Road, Camberwell where staff may attend as needed (this is infrequent) Working patterns: Office based given the nature of the role and having to respond to emergencies and crisis situations within the day.
