Communities Team – To work within a busy Adults Community/Locality team 36 hours per week 9-5 Monday – Friday Needs to be a car driver Experienced social worker Hybrid working mandatory office days Wednesday and Friday and training/duty days Main areas of work Completing Social care needs assessments Support planning Commissioning Risk assessing an manging risks – attending MARAMS safeguarding MCA/BI COP DOLS CHC – DST
