Locum Adult Social Worker Experience of managing Neuro Rehabilitation and Discharge to Assess (D2A) cases Take full responsibility for all neuro rehab and D2A cases Ensure no D2A or neuro rehab cases are left awaiting allocation within the locality teams Support to facilitate discharge for patients who have completed in-patient rehab Queen Mary’s Hospital and Bromley Civic Centre
